DOT Exam Information

Commercial drivers (and non-CDL drivers that operate a commercial vehicle) are required to have a medical certificate.

A DOT physical exam is valid for up to 24 months. The medical examiner may also issue a medical examiner’s certificate for less than 24 months when it is desirable to monitor a condition, such as high blood pressure.

A typical DOT exam takes anywhere from 45-60 minutes to complete.

A few tips and reminders to make your DOT exam as easy as possible:

  • If you wear glasses or contact lenses when you drive, please make sure that you bring them with you to the exam.
  • If you wear a hearing aid, please make sure that you bring it for the exam.

  • When you arrive, you will be asked to provide a urine sample as part of the exam (not for drug testing). Your blood pressure will be measured. Please avoid caffeine and nicotine at least 2 hours before the test as they will rise your blood pressure.
  • If you use a CPAP or similar device, please show documentation regarding its use.
  • If you use insulin, please have all necessary paperwork from the doctor prescribing it and monitoring your condition. The specific form is MCSA-5870. This form is available on the FMCSA website, and the doctor prescribing the insulin must fill it out.
  • Please have a list of all prescribed medications that you take. Also have the contact information of each doctor that has prescribed said medications.
  • If you have been granted a skill performance evaluation certificate, please bring it to the exam. If you have been granted an exemption for hearing, vision, or seizures, please bring proof of that waiver with you.
  • You will need to present a form of photo ID to verify your identification.
